A Thirty-Day Evaluation for New Hires
A new hire evaluation form records what a manager has seen a new employee do in the first month, what still needs practice and what happens next. Owners and managers use it so the end-of-month review rests on observed work, not a feeling.

A new hire evaluation form, in short
A new hire evaluation form is the end-of-first-month record of what a manager actually saw a new employee do, what still needs practice and who follows up by when. The manager fills it with dated observations, and the new hire adds their own view of what is clear or hard. Elvina prepares a sample form for one role with its tasks left as placeholders, and turns each item marked with help into a short lesson and a job aid, available now. She never rates, ranks or recommends keeping anyone.
It describes the present and does not predict who will succeed.
What happens on the free demo
- Book 30 minutes. Pick a time with Elvina's team. The demo is free and runs on Zoom.
- Show the team your goal. Bring one role or one procedure. The team shows how Elvina would handle it, using sample materials.
- Agree on the next step. You leave with a plan for the first step and its price. Nothing reaches your team until you approve it.
When to use it
Use the form at the end of the first month, and again at any later review your own policy sets. It fits roles where the first weeks follow a written plan, because the manager can compare the plan with what actually happened. Give the new hire the blank form a few days before the review, so they can add their own view of what is clear and what is hard.
How to fill it out
Items are the tasks from the first-month plan plus a few working habits someone can observe, such as handing over a job or asking before guessing. Before the review, open the blank CSV form on this page and give each item a row. The sample shows a fictional print and copy shop reviewing a counter associate after a month. Write only what someone saw, with dates.
- New hire and role: who is reviewed, in which job.
- Item: a task from the plan or a working habit someone can observe.
- What was seen and seen on: the specific work and the dates.
- Rating: to standard, with help, or not seen yet.
- New hire's view: what they find clear or hard.
- Next step: practice, a lesson or nothing needed.
- Owner and due date: who follows up, and by when.
| New hire and role | Item | What was seen | Seen on | Rating | New hire's view | Next step | Owner and due date |
|---|---|---|---|---|---|---|---|
| Jordan, counter associate | Taking a large print order at the counter | Quoted and booked three orders alone; checked proofs with each customer | Sept 9 and 11 | To standard | Wants practice pricing rush jobs | Job aid on rush pricing, then one rush order with the shift lead | Shift lead, by Oct 10 |
| New hire and role | Item | What was seen | Seen on | Rating | New hire's view | Next step | Owner and due date |

How to read the result
Do not add up the ratings; read the rows. An item marked not seen yet is a gap in the plan until someone has had a fair chance to watch it. Items done with help need practice and a date. Where the new hire's view and the manager's notes disagree, talk about that first, because it often points to a step nobody explained.
Where a person decides
The manager decides what follows the review: keep the plan, add practice with a date, or take the question to the owner. Decisions about the job itself follow your HR policy and stay with people. Elvina does not rate new hires, rank them or recommend whether to keep anyone. Each item done with help can become a brief lesson and a job aid drafted in Creation Studio, available now. A manager approves them and then assigns them on the Learning Platform.
Who this is not for, and when another tool fits
This is not the right fit if
- You want a score that predicts who will succeed.
- You need a formal performance review tied to pay.
- Nobody has watched the new hire work this month.
Another kind of tool may fit better
- Your HR policy and an HR advisor, for decisions about the job itself.
- A readiness scorecard, if the only question is who can work alone.
